Office Referral Forms

Office Referral Form
The school-wide behavior communication sheet is called an Office Referral Form (ORF). The purpose of this form is to report behavior(s) that need to be changed. Parents will either be called with information of the incident or the white portion of our three-part form will be sent home for a parent signature. This slip must be returned the next day. If the student does not bring back the signed Office Referral Form, there may be further consequences. The purpose of this is to ensure that there is good communication and to build responsibility for the child. If a student receives multiple Office Referral Forms, the parents will be invited to a meeting to problem solve ways to help the student.
